Acknowledgement

If an allergy is listed on your child’s Universal Health Record, a Special Care Plan must be provided to the administration team and all requirements listed (such as an epi-pen) must stay on site at school. *We are a 100% nut free school.

At Bright Seedlings we are proud of our students’ accomplishments and like to show off the work we do to other parents, teachers and students. This includes our website, marketing materials, and our social media platforms.

I hereby give permission for the following people to pick up my child (first visit Photo ID must be present):

I understand that the tuition fee for each month is due by the last day of the previous month for the following month and that late fees are applied on the 1st of each month. (e.g. September tuition is due by 08/30)

I understand that the tuition fee is due at the same amount during the absence period due to vacation or sick leave.

If I am receiving child subsidy, I understand that I am responsible for any co-payments and the period during which child subsidy does not cover tuition.

I hereby give my permission for the administration for of any treatment deemed necessary by the designated physician below or, if the doctor is not available, by another physician and transfer of my child to any hospital reasonably accessible.

Pediatrician’s Information

I understand that by the New Jersey state regulation the following forms/records will be submitted prior to the start date:

1) universal health form and 2) immunization records 3) Medical Action Plans (as above)

By signing the below, I fully acknowledge the receipt of the above-listed forms from Office of Licensing. In addition, I fully acknowledge the receipt of and have read the Bright Seedlings Parent Handbook and will adhere to the Policies stated on the Parent’s Handbook as well.  

I read and understand all policies and rules on the parent’s handbook and agree to it and cooperate with the school. My child is in good health to attend the school, and can participate in the normal activities of the program. If there is any change on the contact information such as phone number or e-mail address, I will promptly inform to the school with written notice. According to the NJ state child care center regulation, all parents have a right to visit any time without notice unless there is a legal restriction.

Our school holds the NJ state child care center license, and follows the NJ state child care regulations. By NJ state regulation, it is our obligation to report any suspected child abuse, neglect to the state Central Registry Hotline (877) 652-2873.
